Personal Care and Rates

At Jefferson Place, we provide 3 personal care service levels. We understand, however, that every resident is unique and that his or her needs may not fit exactly into one of these specific levels, so services are flexible.

To make sure that our residents receive just the right assistance to maintain their greatest level of independence, we provide a thorough assessment. A program of care is then customized to meet each resident's individual needs and adjusted as necessary to accommodate changing requirements.

Care Plans

Basic:
Designed for our more independent residents.

Basic Plus:
For residents who need some additional assistance.

Enhanced:
This level offers a greater level of personal assistance.
